  • Talk with a certified assister with CoverAZ.org to learn about your health insurance options by calling 800-377-3536, or visiting www.coveraz.org/connector

Click here if you are a citizen or business with an Arizona insurance policy and you believe an insurance company or insurance professional violated Arizona law.  The Department does not have the legal jurisdiction to assist with every type of insurance problem.  Our website will help you determine whether you should file a complaint or take a different approach toward resolving a problem.

The National Association of Insurance Commissioners (NAIC) has a great resource called "InsureU" where you can learn about various types of insurance and ways to plan and protect yourself.

The NAIC also has a Consumer Tools and Information webpage, which provides information about insurance companies you can use BEFORE purchasing insurance.

Enforcement Actions

With the Enforcement Actions menu, you can see whether an insurer or professional you are considering using has been the subject of a disciplinary action by the Arizona Department of Insurance.


Insurance Company Rate, Form/Policy, Rule Filings 

SERFF Filing Access (SFA) is an online service that allows the general public to view form, rate, rule, and health plan binder filings that insurance companies submitted to the Arizona Department of Insurance (“ADOI”) in an electronic format, through the System for Electronic Rate and Form Filing ("SERFF"). 


Note: Some of our documents contain an old web address for the Arizona Department of Insurance. The correct address is https://insurance.az.gov.
Form C-TPC: Third-party Consent

Use this form if you want to designate a person to file a complaint with the Department of Insurance on your behalf.
